Along with contact information, timelines, and general moving information, we also include information and locations of further support resources that can assist you with your particular situation.
The binder includes:
- A reader-friendly timeline, outlining when each service component is to take place.
- Detailed outline of each service that is being provided.
- General information that will assist you with creating a positive moving experience.
- Transition Support Service's emergency contact information.
- Staff list of all Transition Support Service people who will be involved with your transition.
- As directed by you, a contact list of all family members, friends, and advocates who have house access and who are to receive regular progress updates. This list also allows you to outline any individual responsibilities or commitments by specific family members during the moving process.
- A signed copy of your Transition Support Services Inc. agreement.
